Island Health is hiring a full-time Intensive Case Management Team Nurse to join the Mental Health and Addiction Services team in Port Hardy, BC.
As the Mental Health & Substance Use (MHSU) Nurse, you will provide core mental health and addictions services including assessment, treatment, rehabilitation, and support for clients in partnership with client, family and community health care resources. In response to an identified client rehabilitation plan, you will develop, plan, implement, and maintain a full range of clinical services and client centered rehabilitation programs. Services are primarily provided in community settings but may also occur in facility settings.
