Pay Rate: $112,622 - $134,477 Annually

The Toronto Grace Health Centre (TGHC) provides specialized care to individuals requiring Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional Care, and Palliative Care services. Our focus is on supporting recovery, maintaining function, and enabling safe transitions across the health care system, whether returning home or transitioning to Long Term Care (LTC).

The Toronto Grace Health Centre (TGHC) currently operates across two sites: the main downtown location at Church and Bloor Street, and the Scarborough site at Victoria Park and McNicoll Avenue, which houses transitional programs. POSITION SUMMARY: 

Reporting to the Patient Care Manager, Toronto Grace Health Centre is currently seeking a Nurse Practitioner for its Scarborough location. The Nurse Practitioner is an active member of the interprofessional team. This role brings strong clinical expertise and leadership in the care of older adults with complex medical needs, including frailty, dementia, and chronic disease management within a transitional care setting. The focus is on supporting functional recovery, optimizing quality of life, and facilitating safe, seamless transitions across the continuum of care. 

The Nurse Practitioner serves as a clinical leader and mentor to nursing, allied health, administrative, and support staff, promoting best practices in seniors’ care and fostering strong interprofessional collaboration.

MAIN RESPONSIBILITIES: 

• Provide patient- and family-centred care for older adults with complex health needs, both independently and in collaboration with the interprofessional team
• Conduct comprehensive geriatric assessments, including physical, cognitive, functional, and psychosocial domains
• Develop, implement, and evaluate care plans that support functional maintenance or improvement, symptom management, and quality of life
• Support safe and timely transitions in care, including discharge planning to home, community, or long term care settings
• Collaborate within an interprofessional shared care model to ensure continuity and coordination of services
• Promote and apply best practices in seniors’ care, including the management of frailty, dementia, delirium, and chronic conditions
• Provide clinical consultation and leadership to support complex decision-making
• Engage patients and families in advance care planning and goals-of-care discussions
• Liaise with internal and external partners to support integrated service delivery and patient flow
• Contribute to quality improvement initiatives focused on seniors’ care, education, patient outcomes, and experience 
• Support and mentor staff through education, coaching, and role modelling of geriatric best practices
• Ensure care delivery aligns with professional, ethical, and legal standards
• Promote a safe, respectful, and high-quality care environment 

QUALIFICATIONS & EDUCATION: 

• Currently registered with the College of Nurses of Ontario as a Registered Nurse in the Extended Class
• Current membership with RNAO or NPAO (or equivalent)
• Completion of a Primary Health Care Nurse Practitioner Program and a Master’s degree in Nursing
• Minimum three (3) years of relevant clinical experience, with a strong focus on geriatric care, complex continuing care, or transitional care
• Demonstrated experience working with older adults with frailty, dementia, and complex chronic conditions 
• Additional education or certification in gerontology, seniors care, or dementia care is strongly preferred
• Experience supporting patients through care transitions, including discharge planning and community integration
• Strong clinical knowledge in chronic disease management, geriatric syndromes, and functional assessment
• Demonstrated leadership, collaboration, and communication skills within an interdisciplinary environment
• Commitment to quality improvement, evidence informed practice, and ongoing professional development
• Demonstrated use of proper body mechanics when completing job demands such as mobility, transfers, and safe patient handling.
• Must submit evidence of COVID 19 vaccination requirements as per Health Canada and organizational policy. Medical exemptions or any other kind of requested exemption based upon the Hospital’s obligations pursuant to the Ontario Human Rights Code will be considered on a case-by-case basis.
