About the Opportunity
The Nurse Practitioner provides health care service as a member of the collaborative team that includes Clinical nurse lead, Occupational and Physical therapists, Wellness Navigators, Social Workers and Care Coordinators. The Nurse Practitioner (NP) provides comprehensive clinical care to increase access to equitable primary care to community members who are unhoused and to improve the health status of marginalized and underserved populations. To create a collaborative environment in which information, knowledge and resources are shared among team members. The NP partners with and complements other members of the health care team for the achievement of person-centered care. The NP as the most responsible healthcare provider (MRHCP), works autonomously to diagnose, order, and interpret laboratory tests, order and interpret reports of diagnostic imaging investigations, prescribe pharmaceuticals, and perform procedures. The NP provides direct care and partners with other members of the health care team for the achievement of the most appropriate and timely care for patients. working autonomously or in collaboration with other members of the team, the NP provides care for and monitors the physical/biologic, psychologic, and spiritual needs of patients and integrates these into holistic care plans engaging as appropriate with family, community, and other agencies/services.
The NP adheres to the NSCN Standards for Nursing Practice and the Code of Ethics and is responsible and accountable at all times for their own practice consistent with relevant legislation, and professional, and ethical standards. The NP is accountable to identify, analyze, and evaluate activities that are reflective of quality improvement initiatives.

$59.46 - $69.52 Hourly
$71.95 with 25 years of NP experience
Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
